Back pain is one of the most common reasons people in Ireland visit a GP or take time off work. Sometimes the pain stays in the back; sometimes it travels down a leg, which is often called sciatica. Knowing the difference helps you understand what is going on, what helps, and when to get checked urgently. This guide explains both, then gives an honest view of where CBD does and does not fit.

Back pain vs sciatica

Non-specific back pain Sciatica
Where it hurts Mainly the lower back, sometimes buttocks Pain travelling from the buttock down the back or side of the leg, often below the knee
What it feels like Ache, stiffness, spasm Shooting, burning or electric pain; may have numbness or pins and needles
Usual cause Strain of muscles, ligaments or joints Irritation or pressure on the sciatic nerve roots, often from a disc bulge
Typical course Most improves within weeks Often improves over weeks to a few months

Leg pain is often worse than the back pain in sciatica. Both can exist together.

Red flags: get checked urgently

Contact your GP or go to an emergency department immediately if you have back pain with any of the following:

  • Numbness around the genitals or back passage (saddle area).
  • New difficulty passing urine, or loss of bladder or bowel control.
  • Weakness in both legs, or a leg getting weaker.
  • Fever, unexplained weight loss, or a history of cancer.
  • Pain after a significant fall or accident.
  • Severe pain at night that does not ease with any position.

These can be signs of cauda equina syndrome or other serious conditions that need urgent treatment.

What helps most

Keep moving

Bed rest slows recovery. Gentle walking and gradually returning to normal activities, even if they are uncomfortable, is one of the most effective things you can do.

Exercise and physiotherapy

Exercise programmes tailored to you, guided by a chartered physiotherapist, have good evidence for back pain and sciatica. Many people in Ireland can access physiotherapy through the HSE (with a GP referral and sometimes a wait), private practices or health insurance.

Pain relief to keep you active

Over-the-counter painkillers, used as directed and with your pharmacist’s advice, can help you stay mobile. Your GP may suggest other options for sciatica if pain is severe.

Heat

A hot water bottle or heat pack can ease muscle spasm for some people.

Sleep position

A pillow between the knees when lying on your side, or under the knees on your back, can ease discomfort.

Reassurance

Most back pain and sciatica improve. Worry and fear of movement can make pain worse, so understanding that hurt does not always mean harm is helpful.

When surgery is considered

Most people with sciatica recover without surgery. Surgery may be considered if severe leg pain lasts for months despite treatment, or if there are signs of nerve damage. A specialist will discuss risks and benefits.

Where CBD fits

There is no good evidence that CBD supplements treat back pain or sciatica. Studies on cannabis-based products for back pain have mostly involved THC-containing medicines, and results have been modest and mixed. Some people include CBD oil or a CBD balm in their routine, for example massaging a balm into the lower back, and find the routine itself helpful. If you do, keep up the movement and exercise that make the biggest difference.

What the law in Ireland says

CBD oils, capsules and gummies are sold in Ireland as food supplements, overseen by the Food Safety Authority of Ireland. Food supplements are not allowed to claim to prevent, treat or cure any disease, so no CBD supplement can legally be sold as a treatment for back pain and sciatica. Medicines are regulated separately by the Health Products Regulatory Authority (HPRA), and cannabis-based medicines are available only on prescription in limited circumstances. If a seller tells you their CBD treats back pain and sciatica, treat that as a warning sign about the business.

If you decide to try CBD

  1. Speak to your GP or pharmacist first, especially if you take any regular medication. CBD is processed in the liver and can change the levels of some medicines, including blood thinners, some anti-epileptic drugs, some antidepressants and sedatives.
  2. Keep your proven treatment going. CBD should sit alongside the care your doctor or physiotherapist recommends, never replace it.
  3. Choose a lab-tested product with an independent certificate of analysis, CBD stated in milligrams and THC not detected.
  4. Stay within the recommended daily serving on the label, and keep the amount steady for a few weeks rather than changing it daily.
  5. Keep a simple diary of symptoms, sleep, activity and what you took, so you can judge honestly with your doctor whether anything changed.
  6. Stop if you notice side effects such as drowsiness, stomach upset or appetite changes.

Choosing a CBD product

Oils give flexible amounts by the drop, capsules and gummies give a fixed amount, and creams or balms are applied to the skin as part of a massage routine. Whichever you choose, check for an independent lab report with a batch number that matches your product, a clear THC result, a full label and a seller with real contact details in Ireland or the EU. Our guide to the best CBD oil in Ireland explains each check, and CBD and pain: what the evidence says covers the wider research.

Safety

  • CBD is not recommended during pregnancy or breastfeeding, and CBD food supplements are not for children.
  • Do not combine CBD with alcohol, and do not drive if it makes you drowsy.
  • Do not apply creams to broken skin or wounds.
  • Products containing THC can affect driving and drug tests. Only use products whose lab report shows THC is not detected.

Our position

BDI.ie is run by the team behind two Irish CBD brands, Dr. Hemp Me and CBD Éire. We sell CBD as a food supplement and we do not claim it treats back pain and sciatica. If you live with back pain and sciatica, the most useful thing you can do is get a clear diagnosis and a plan from your GP or physiotherapist. If you want to include CBD, do it with their knowledge and with realistic expectations.

A simple four-week diary

Whatever you try for back pain and sciatica, a short daily note makes it far easier to judge honestly. Each evening, score your back pain and sciatica from 0 to 10, note your sleep, activity, any medicines or supplements taken and anything unusual about the day. After four weeks, look back with your GP or physiotherapist. Patterns often show up that memory alone would miss.

Back pain at work

Desk workers, drivers, farmers, carers and tradespeople all face different back pain risks. Change position often, set up your chair and screen so you can sit upright with feet flat, take short movement breaks, and use good lifting technique with the load close to your body. If your job is physical, ask your employer about manual handling training and adjustments while you recover. Staying at work, with modified duties if needed, usually helps recovery more than staying at home.

If pain is stopping you from working, your GP can certify time off and suggest a phased return.

Small, regular movement breaks matter more than perfect posture.

Frequently asked questions

Does CBD oil help back pain?

There is no good evidence that CBD supplements treat back pain. Movement, exercise and physiotherapy have the strongest evidence.

Does CBD help sciatica?

There is no good evidence that CBD treats sciatica. Most sciatica improves over weeks to months with movement and time.

How long does sciatica last?

Many cases improve within six to twelve weeks, though some take longer. See your GP if it is not improving.

Should I rest with back pain?

No. Gentle movement and staying as active as possible helps recovery more than bed rest.

When is back pain an emergency?

Numbness around the genitals or back passage, bladder or bowel changes, or weakness in both legs need urgent medical attention.

Can a CBD balm help my back?

Massage can feel soothing, but evidence that CBD absorbed through the skin reaches deep tissues is limited.

Can I take CBD with painkillers for sciatica?

Ask your GP or pharmacist; CBD can interact with some medicines, including some used for nerve pain.

Do I need a scan for back pain?

Most back pain does not need a scan. Your GP will advise if your symptoms suggest one is needed.
